Registering With The Practice
You may register with the Practice if you live within our catchment area. Just bring your medical card to reception. If you do not have it, don’t worry, we will ask you to fill out a special form and can still accept you. It helps if you can bring details of your previous doctor with you. New patients will be asked to make an initial appointment with our nurse for a health check
Patient Choice
Please tell the receptionist if you wish to see a particular doctor, otherwise you will be offered the first available consultation with any one of the doctors. We will always try to accommodate your preference, but please understand that sometimes it may be necessary to wait longer for an appointment with a particular doctor.
Repeat Prescriptions
The practice has a computerised repeat prescription system. With the doctors agreement, you may obtain repeat medication by handing in, or faxing to 0118 940 1357, a request slip for collection of prescriptions 48 hours later, or by sending a written request enclosing an SAE for posting. Please ensure that all handwritten requests contain the following information: Name, DOB, the name of the medication, the amount/dosage and how the medication is taken. For safety reasons, we only accept requests for repeat prescriptions over the phone in exceptional circumstances. If you live over one mile from the chemist we can dispense your medicines. These can be collected from the main surgery or delivered to Knowl Hill and Waltham St. Lawrence on the days of the branch surgeries. Repeat Prescription Requests via E-mail
Wargrave Surgery is now accepting repeat prescription requests via the email address
bw-pct.Wargravescripts@nhs.net. When making a request please ensure that you state your name, address and date of birth. In addition please provide the following information about the medication you require; name of the medication, the dosage of the medication and how it is taken.
Prescriptions will be ready to collect at the surgery 2 working days after receipt. If you send your request at 2100 on Monday it will be ready to collect on Thursday at 0800. A request sent on Friday at 1500 will be ready to collect on Wednesday at 0800.
Sickness Certificates
The doctors can only provide a free (National Insurance) sick note for patients they have seen, who are off work for a week or more. For periods of less than 7 days your employer should provide you with the form for “Self Certification” of sickness. A fee will be charged for certificates for less than 7 days and for non-NI certificates.
Investigation Results
Please telephone for test results after 11am.
