The surgery cannot offer ad-hoc urine cultures or blood tests; they must be preceded by a consultation with a clinician who will provide a diagnosis and determine the purpose of the tests to be conducted.
All Urine culture and blood tests must be accompanied with the test request forms, (these will take the form of entered on to your GP medical records by your consulting GP) or the forms handed to the patient by the consultant or specialist from a hospital visit.
